Visa information


All international students who intend to study at NCU Collegium Medicum are required to obtain a document which proves that their stay in Poland is legal (registration of residence).

The residence registration procedure depends on the student's citizenship. Here's a list of what you need in order to apply for residence registration:

Students from countries which are members of the European Union + Norway*:

  1. application form – must be completed in Polish; it can be downloaded here: EU citizen residence registration
  2. copy of passport – the page with the photo (the original is to be shown at the Office)
  3. copy of the European Health Insurance card (the original is to be shown at the Office)
  4. confirmation of being a Collegium Medicum student (it may take up to 5 days for the documents to be signed)
  5. confirmation of the student’s ability to support himself/herself in Poland (bank statements from the student’s bank account (in Poland), loan granting confirmation, income statement)
  6. no payment/stamp is needed.


*Due to agreements between Norway and EU, citizens of Norway go through the same procedure as students from EU countries.

Students from countries which are not members of the European Union:

  1. application form – must be completed in Polish; it can be downloaded here: non-EU citizen residence registration
  2. 4 colour, valid photographs of the student - taken within past six months, size 35x45 mm:
    • photo showing the eyes and face from the top of the head to the shoulders, so that the student’s image fills 70-80% of the entire frame
    • photo taken on a plain light background, in frontal position with open eyes and looking straight ahead, with a relaxed facial expression and closed mouth, presenting natural skin colour
    • eyes (especially pupils) need to be clearly shown, the lines of the student’s eyes should be parallel to the upper edge of the photograph
    • photo taken without any head cover - NOTE: Students who wear head cover according to their religious principles may have the photographs taken with the head cover, but the face must be visible (in such a situation students must provide statement of belonging to a religious community)
  3. copy of passport (the original is to be shown at the Office)
  4. confirmation of being a Collegium Medicum student (it may take up to 5 days for the documents to be signed)
  5. proof of valid insurance coverage
  6. bank statements from the student’s bank account (in Poland) confirming that the student has the financial means to cover his/her stay in Poland
  7. confirmation of covering the costs of issuing the temporary stay card (340 PLN) and the costs of the card itself (50 PLN)
